This is what I did and it solved the problem. I don't know which step worked so I just tell you everything I did.
1 load a save very early and go straight to the leper's smile. save the traveler and heal him
2 load an auto save entering leper's smile then change the difficulty to story and heal him
3 load an a save just before leper's smile stay on core difficulty not touching anything and heading straight to crevice as soon as possible and save him. heal option still there.

just do not load any quick save AFTER entering leper's smile. It is a waste of time. took me two hours to find out.
Aye, i loaded a save before i went in, and then went straight to the crevise and magically it worked. I did switch out Embar for Daerun though, so im not sure if that impacted the result.
Yeah, I think you actually need a Cleric in the party, not just any character that is able to heal.

I always been able to do it with Ember and Daerun in my party.
But after the patch no longer possible.

Look like I need to mess with the party comp :steamfacepalm:
After putting the Sosiel in my party it finally showed up.
Look like for some reason Daerun in my game can't do it anymore or ember.
Hey, I was thinking I had the same problem, but it finally looks like it's more a matter of what you are doing on your way to the queen and not only about your group composition.

If you're not bringing Sosiel with you, you will need to loot at least one group of bodies on your way to the queen to get the option to heal the halfling (loot until you get the quest notification about the rest of the soldiers still battling the ennemies and dying). You will still need to either be a healer yourself or have one in your group (from what I read, Camelia, Daeran and Ember all work, Seelah doesn't).

If you go straight to the crevice to save the lost traveler, you won't have the option for healing him , even if you're a healer or have one in your group. It took me an hour and several tries to figure it out and check it.

Weird. I do believe you, though I just the thing just yesterday, went straight to the halfling and it was no problem to heal. Might be worth the bug report then.

you probably had the cleric.
I use to be able to run straight to it and rescue him with daerun or ember instantly.
at least now I know how to go around the bug.
Hey folks,

Its not a bug, you have to be able to use a "Cure Serious Wounds" spell in order to heal the lost traveller. I was unable to heal him even with Daeran in my group which I leveled up to 7 when I entered the Leper's Smile. Naturally, I didn't have level 3 spell use since I didn't rest. Then I tried to level up Daeran to 7 by picking heal spell and rest, I was able to heal lost traveller right after. Its just like "Glitterdust" spell use option in Kingmaker where you encounter lizard tribe "Elder" soul and reveal him, having the spell available gives you dialogue option.

Imo there is checkpoint, they even warn You about it on the very beggining via Anevia, You either go right and get A(choices) or go longer route for B(loot) and loose choices as trade-off.

That definitely not it as I already did have cure serious wound.
I already know the problem as I did what the other person said and it worked.

Hey,

Having the spell is not enough, you must be able to cast it too (having daily use). I have confirmed this as I checked the spell right before and after the encounter. I had 5/5 before and 4/5 after the encounter. Having daily use is the strict condition, but I don't know if it is character specific. Feel free to check it yourself.

Seriously dude?
I did have spell charges for it.
hey guys I think I've figured it out, In case anyone still feels confused about this, here is the conclusion: actually, the KEY to save that lost traveler, is Cure Serious Wounds. spell, potion, and scroll, each one of them will make things correct. sometimes looting bodies will work but I think this is just because what the first group of bodies carried is 2 Cure Serious Wounds potions, coincidentally. and it's worth noting that any spell higher or lower than Cure Serious Wounds will not work at all. this is really really stupid unless I was wrong.
