It's the siamese twin skeleton you can find in the abandoned circus wagon near Manzanita Post. As Jed said, is a cheater trick....

IIRC, the two-headed skeleton is a prop in a circus freakshow lying in a cage in single player.

Hackers just applied the prop to an NPC enemy body to attack....

hacker that spawned the 2-headed skeletons near me was talking and being somewhat entertaining or at least trying to be rather than instantly killing everyone repeatedly...
they did not offer any
explanation about where the skeletons were from, but they let us play with them for awhile. this hacker himself was not disguising his character as an NPC skeleton body nor were the hackers that spawned...
