For me I think a shot in the head would be fatal, if no helmet would be on, or getting totally swiss cheeced by bullets, or heavy explosives.
If you'd have a medi helo, one could stabilize certain wounds on casualties while they would ship them over to a hospital. And once you got there, you could respawn.
I think having a base fulled with the vehicles you could use wouldn't work, since someone would come up and blow them up, but maybe just having a counter as to how many vehicles all in all one side could use would work. And you'd need to be certain profession to use them or something, to limit eveyrone just wasting vehicles.
The side that loses all it's lives would lose the war, and one could capture towns to gain certain benefits maybe.

And what would that be without a drill sergeant mod automatically turning your pc on in the morning and screaming at you.
You should also be able to plug your coffee machine and over into it so you'd at least get the benefits of a ready made breakfast.
But in all seriousness, a lot could be done to counter the craziness in Arma 3 with some of these suggestions, and also to make it mroe awesome to play as a full squadron.
Or even better, if you die in the game, you die in real life.
