Medic's head cant be removed.
only pyro's head can be. other medic head replacements just cover the head completly

But you can resize it and make it so small but idk if it will work

You can but that can only be done through server commands. It would not work in the way you would think.

also, when a playermodel has its head shrunk, all cosmetics attached to the head will shrink as well.

I believe it should be able to be done at least, maybe more difficult but it is possible. They use that same exact technique on the Eyelander when it beheads people (if you look at your decapitated corpse neck long enough you'll notice).

The head is shrunk to a negative value for that to happen and that's a animation. Cosmetics cant give player models animations.
