I love the idea of this it adds to the spy being a spy with tricks up his sleeve.

I would like to see animation or the spy clicking the pen and then a sharp pick would protrude out of it. That would be sick!

no damage when non backstab, does not break disguise when backstab(but does on miss or normal hit), enemy does not die immediately, instead takes 100 health per second for 5 seconds starting 2 seconds after backstab(poison damage so they know it is a spy) so have 2 seconds to get away before anyone starts dying from your backstabs and hence you cankill a lot of people before anyone notices, however any healing whatsoever ends the poison effect

I like the face stab idea... Blind them then maybe give the victim the look of jarate but black. Or: On back stab disguise the victim as the spy for 10 sec with a bleeding health drain and instantly cloak without sound.

Front-stabs blind victim for 5 seconds.
-10% damage done.
-5% max health on wearer.
