A hood, scarf, and gas mask for the pyro, all in one item. The hood and scarf are both fully paintable, as shown in two of the pics.

Also, the white clipping on the sides of the gas mask isn't actually part of the item. I had to also equip the apparition's aspect to avoid the pyro's normal mask showing through mine, and the white slits are a tiny bit of clipping from it. Just pretend it isn't there, because it won't be.

EDIT: I've added some variation to the color of the hood and scarf so that it better fits into the TF2 artstyle. Pictures updated.

NOTE: Since I've seen a number of people speculate on the inspiration for the item (mainly Quarians from Mass Effect), I guess I might as well clear it up. First, it is not inspired in any way by Mass Effect, since I have never played it nor have I seen it be played, though after I looked up the Quarians I have to say that it's a very surprising similarity.) The item was actually inspired by two different things. The hood and scarf are based on the hood worn by Malzahar in League of Legends, and I was actually going to have the end of the scarf draped over the pyro's left shoulder before I scrapped that. After I made the hood, I decided to add a gas mask instead of my original idea, and so I modeled the mask loosely based on the helmet worn by Desert Punk in the show of the same name.
