Gem Locations

There are 21 gems scattered throughout the Drevis Residence. Collecting them all unlocks a special scene after the credits. While searching for the gems, make sure you complete the steps necessary to get the True Ending, and then proceed as usual.

1. Nursery, left side of the bookshelf.

Upon inspecting the left side of the bookshelf a gem will pop out on the ground.

2. Entrance Hall, at the very bottom.

Watch out for the zombies walking around the gem!

3. Archives, return the pendant.

In the Archives go to the right side of the top left shelf and read the book. After the cutscene with the little girl leave the room and walk back in. There will be a pendant on her place.
Pick it up and go to the Bedroom, next to the Nursery. Go by the window and use the pendant to trigger another cutscene after which you will recieve a gem.

4. West Passage, follow the zombie.

In the West Passage, the area outside the Archives, follow the zombie into the wall. You will find yourself in a small dark room with a gem in the middle.

5. Doll Room, on the throne.

In the Doll Room simply walk forward. The gem is sitting on the throne. No mystery here.

6. Courtyard, the gate on the right side.

After moving the painting in the Reception Room and going out on the Courtyard follow the pavement to the metal gate.

7. Cafeteria, behind the curtains.

In the Cafeteria move the curtains to reveal a dead body. Upon walking away, the body will fall, dropping the gem.

8. Kitchen, in the sink.

In the Kitchen where the dog is walk around the stove and go to the sink. Upon inspecting it a gem will pop out.

9. Cultivation Room, after the cutscene.

At the end of the cutscene Aya will run after her father into the next room. Walk back into the room where the cutscene took place and collect the gem.

10. Mortuary, lead the body to his head.

When you enter the Mortuary there will be a head on the ground in the top left side of the room. Talk to it and walk upwards towards the next room. When you are near the door you will hear a sound.
Go back to the lower side of the room next to a fallen body. It will stand up and start following you. Simply lead the body towards the head and you will be rewarded with a gem.

11. Attic, the barrels on the left.

After dropping the box from the Attic and obtaining the mini-chainsaw climb back up the ladder and destroy the barrel in the upper left corner of the room.

12. B1F Passage, destroy the barrels.

The Passage is the room after the Mortuary. There is an old man sitting on the ground in the left side of the room with a couple of barrels next to him. Destroy the top barrel with the mini-chainsaw to obtain the gem.

13. Spare Room, return the eyes.

You will be awarded a gem after returning the eyes to the girl with golden braids that has locked herself in the Spare Room.

14. B2F Passage, destroy the barrels.

Use your mini-chainsaw to destroy the barrels next to the crow.

15. Laboratory, the broken glass case.

Release the big guy with your mini-chainsaw. He will trash the laboratory, breaking the glass case which cointains the gem, allowing you to pick it up.

16. Hidden Passage, near the doll.

From the Jail you enter a Hidden Passage. When you reach the second doll that is looking around, go to the upper right corner of the room. Pretty hard to miss.

17. Jail, release the prisoner.

Upon making your way through the Hidden Passage with the dolls you will find yourself back in one of the Jail cells. Take the key on your right and exit the cell.
Release the prisoner from the cell right in front of you and you will be awarded with a gem.

18. Mirror Room, the empty wall.

After the Jail you enter the Mirror Room. To get the gem go to the wall on the opposite side of the mirror and interact with it.

19. Big Passage, by the fallen rocks.

On the right of the doll that's looking around. Pretty obvious.

20. Private Room, the stuffed dog.

After you cut up the dog with the scalpel and get the Incinerator Key the mandrake will be walking around with a bone in its hand. Take the bone from it.
Go back to the Private Room and give the bone to the stuffed dog. A gem will pop out.

21. Cultivation Room, glowing mandrake.

Back in the Cultivation Room the mandrakes will have broken out of their glass cases. In the little room, where you solved the Aya puzzle, there will be a glowing mandrake. Cut it up with your mini-chainsaw and it will drop the final gem.

I found a guide for the remake. I'd leave this version as it is since the original version of the game is still playable via the launcher and this is good guide.

Nope it isn't. At least not for the 2020 remake version. The one that you find in the bathroom isn't listed here for example.
