why is the game different when you pick Training

I typically play on Medium. I prefer the golden middle way. And I know about the difficulty. That's one of the reasons I stopped. But I myself had no trouble surviving. It was actually the NPCs who were a bitch to protect.

It can be very difficult at times. The mapper understands the tactics that players commonly use and employs ways around them. If you typically play on Hard, I suggest playing this on Medium difficulty. Its far more enjoyable.

I've tried to beat this mod several times, but never had the nerves to get there.
On the other hand, I beat Chemical Existence, so I should be able to finish pretty much anything...
