I'm not sure how much this is related to that overall matter, but I never had an option to adopt a child until I got the Hearthfire expansion. I was given a message about buying land then a bit later another message about adopting a child. I began building a home in Falkreath during the Dawnguard and Dragonborn campaigns. When I finally had enough room and furniture for kids, I was able to adopt her. Can't adopt a kid if you don't have room for them.
I waited until I finished Dawnguard and Dragonborn before adopting a child though. The threat from vampires and Miraak's servants was enough of a threat for me to consider holding off adopting until I finished those campaigns.
I have to say, having lost my own daughter ... hearing a child call me poppa for the first time touched something deep inside me ... deep.
I'm quite certain there are children around every big city since I started Hearthfire. They're mostly orphans because of the war. There is a young girl around Windhelm who goes around selling flowers out of a basket. (CAP mentioned her by name; "Sophie"). If I had room then, I could have and would have adopted her on the spot. I still may go back and find her. Lucia would then have a sister to play with.
