I'm not sure, but you may also have to wait awhile before they show up. Try sleeping in an inn or "waiting" a day outside of the zone that they are in and see what happens.
Do any of those conditions apply? Have you installed the "Unofficial Hearthfire Patch?" (I really recommend all the "Unofficial" patches by the Unofficial Team. You can get them at nexusmods.
Just run through the console commands and see if you can force her to join the adopted/whatever faction. Or, reset her to her original state, then run through the adoption process again.

There should be a combination available that will put her back on the right adoption track.
Try disabling the Hearthifre DLC in the mod list and running the game without it for a while, then re-enabling it which should reset HF. Obviously not a good idea if you've already built a HF house or two.
