The achievements don't unlock if you launch Skyrim with certain mods installed, and since the game is so much better with mods, you can't fault people for never having played it without mods installed.

Mods in general don't block achievments.

Using the command menu does with certain commands I think but mods shouldn"t.

The console also DOES NOT block mods. its impossible to use the console to meet the requirements of some achivements, but the console itself doesnt interfear.
Neither do mods

Never had not unlocking achievement and I run it with over 150 mods skse and so forth. Most console commands don't prevent unlocking however giving yourself artifacts or some stuff will not update quests or give you achievement for instance giving yourself daedric artifact by console.
