If you do not have mods, or sorting the load orded did not help, then make sure that your official master files are not damaged.

I am surprised that validating game files with cleaned appcache did not solve this.

(that one I guessed, it's probably not the correct one, just an example). If you find the relevant code for the tunnel then you should be able to use "coc" to basically spawn just inside the door (*probably* the door at the end of the tunnel you're at now, *possibly* at the other end, in which case I'd recommend reloading to before trying it as this could confuse the game further.)
