right click on skyrim in your list of games, left click on PROPERTIES, left click on LOCAL FILES, left click on VERIFY INTEGRITY OF GAME CACHE,,,,when it is done-don't worry about what it says-just close down that page and press PLAY

Tab doesn't work at all/broken somehow, get a new keyboard. If somehow the function has been changed, hit escape, controls, scroll down to character menu, click whatever is there, and at prompt use the Tab button to make it default again.

could be a quest triggered thing or are you just doing your business, etc wandering around town or the wilderness.

Have you tried completely reinstalling the game?
If that doesn't work, check the key binding menu in the game, see if everything is bound properly.
Also, if you have a controller connected, that could be your problem.
