Controller remap glitch

When I begin to play with my wired x360 controller, everything is fine, but eventually, the input halts for a moment, and when it resumes, A is mapped to X, X to Y, Y to A, start to the left analog click, and so on. I've tried to use x360ce, but the same issues occur. Any known fixes?

also same issue Considering the game doesn't actually give you a way to map controls its weird that its even possible for it to do this.
