The +1, +2, +3, and +4 give stat boosts when you create an item with fusion. The numbers are directly added to the item's highest stat. Multiple bonuses stack up so, if you find an ingredient with a +4, and fuse it with an ingredient with a +3 bonus to make an item, the resulting item will have a bonus of 7 points added to its highest stat. The bonus is capped at +15 though, so if you fuse 5 items together which all have a +4 bonus, the resulting item will have a +15 bonus rather than a +20 bonus to its highest stat.

So that being said, you should avoid throwing away any items which have a +4, unless it is something really common, or that you know for sure you will not need for fusion.

Items with '+'s have stronger effects (heals more hp/sp, more damage, better protection, etc). When used as ingredients in Fusion, they add up the total +'s to determine the final value of the item created.

Thus level 5 fusions with high quality components will yield significantly more powerful equipment. The challenge is getting them into the hands of adventurers that would rather buy candy. Putting the equipment out on the tables is a quick way to see the wrong people buy it up.
