ideally the majoirity of your builders need to be atleast level 60. run glitterhelm insane hardcore camapign to level your toons to atleast 60 the more the better. try to break the 200-300 barrier
you can complete a challenge known as raining goblins on nightmare with a level 60 squire with high end godly gear. the reward is a very useable staff for the mage. ogre crush is another challenge to obtain the ogre party hat for a dps monk.

tl:dr version, collect all of one type (leather/chain/etc.) in godly and give them to your characters, then do the same with mythic.
The higher Maps (Endless Spires, Summit, Glitterhelm) will bring you more Myth´s than Deeper Well. Because of the ammount of ememies that will spawn.
The Item Sets are also a good way to improve your Stats. But remember, that you can not mix Godly with Mythical Stuff in one Set.
Another way to catch up some Mythics is to do the first wave on Endless Spires NMHC. The Wave is easy to do for a Tower App even without high stats. The Chests after the First Wave will give you some Myths that can upgraded up to 90.
Another thing is that you need some Familiars for all your Charackters. On active Builders (like Squire or App) a Genie would be nice (try to beat wave 15 on Hall of Court on Hard or even better on Insane HC... MM for a much better Genie). On inactive Builders (like Hunter or Monk) you will need a Familliar that boosts your Stats. For Example you could use the "Giraffe". This Famillar is awarded after beating wave 25 on Deeper Wells.

What I did: One after the other I raised my heros to level 74 doing (for the ep) Glitterhelm on insane, City in the clifts on medium, King's Game on medium and later hard.To get mana I played some maps on survival (medium and hard). With the mana I went to the shops and bought some decent gears.
Things started to improve after the first hero reaches 74 and had a full set of mythical gear of the same type. This hero (apprentice) gave the towers that helped the other heros to reach also level 74.
But be careful you have to invest a certain amount of time in the shops. More often than not mythical gears in the shops are overpriced and undersexed. There are some shops that sell good mythical gears (around 300 and more tower points together) for 5 - 12 mil; other sell crap for 50 (and even more) mil.
When you can do survival on insane you have a fair chance to get even some mythical gear (which is not good enough, though) and useful pets an weapons.

PS: One more question Insane HCMM... Hard Core MM? Mix Mode? I wasn't aware MM boosted gear stats by that much.
last i heard it was like a 30% increase in difficulty for 5% increase in drop quality,if you cant handle mix mode then it is totally fine to turn that mode off, as the quality increase is small. but it does make the survival alot quicker.
towards later waves of insane survival mythical quality loot will begin to drop, i have personally seen myth loot on maps as low as foundaries and forges.
