If you're at all familiar with Killing Floor, it's not like that game where you can join at the instant the Patriarch is killed and get the map achievement without ever even selecting a class let alone playing it for a little. You could even sit in spectator and get the achievement. L4D2 isn't like that; L4D2 is done right.
I don't think you need to be present in the opening sequence, I think you just need to be around in the first chapter of the map. Not sure if you need to survive the ending, but just to be safe, make sure you create the game and survive the ending.
