L4D2 infected are pretty balanced at the moment as the game is (except for the charger with 10-damage melee hits and opportunities for instant death). I can't see Valve bringing in more special infected without ruining the balance of the game.
The survivors that we currently have from L4D1 and 2 could be used again if L4D3 came out (but obviously Bill would be dead). There have been suggestions that Keith (Ellis' friend) is brought in, but guessing from the stories that Ellis says in-game, Keith became infected after the "99% zombie proof van" failed that him and Ellis built (only guessed, but for some reason Keith and Ellis were separated, most likely because Keith was killed or became infected). The Midnight Riders have also been suggested, but as mentioned in the graffiti on the walls across L4D2 maps, the Riders were evacuated early on so it wouldn't be feasible for them to be playable in campaigns.
They could do new maps, exploring encounters with other survivors perhaps like many of the modded campaigns from the workshop have done. Fighting for equipment, perhaps exploring the "Room for One" mutation, with an 8-player campaign in two separate groups both heading for the same rescue vehicle that has limited space.
The only other thing I can think of is introducing vehicles that we as survivors can use, plan our own routes through various options. Have US fly the helicopters, pilot the planes and steer the boats. However, this sounding really fun, it would be almost impossible to do without having set "happenings" like plane crashes or running out of fuel that would mean the survivors would have to stop and run through hordes etc. like we have to now.
Unless Valve come up with a huge change more than graphics then I can't see L4D3 even be worth paying for, and I am sure lots of people will agree with me.
a basic story, 4 people, most likely a black person and a female with 3 males.
some cool weapons randomly put through the world, and maybe 2 or 3 new special infected. just like both games had before.
