Not a bad idea.
I'm assuming you mean the part of the hospital with the elevator crescendo event?
Except, with such a game mode as survival - the amount of common, SI and Tanks in such a confined space would quickly overwhelm the players.

Unless you mean the open area closer to the beginning of NM-4 just after you go up the stairs near the safe room, and then through a short hallway ?
That would probably make for a better survival map, but still be quite challenging.
Would be fun to try though!

I saw a mod but for L4d1, where it is no mercy but backwards. start at the roof and I gues the apartment is the finale.

Hmh, that would be kinda cool - begs the question why they're on the Hospital rooftop I guess, but it would kinda be like the Dead Centre Hotel map intro.
