Mine is Bind alt c_thirdpersonshoulder.. and I can switch back and forth in game instantly.
I just put the crosshair over the gun and high so nothing blocks the view
Worst thing is when your surrounded headshots wont be too great until your use to it.
The mods don't help. Doing it manually is best.
I would say test with the offset and height until it is to your liking.
cam_idealpitch # (Leave at 0 or the crosshair won't match the shooting direction)

cam_collision # (Whether the camera collides with walls so as not to see through them.)
cam_ideallag # (How much the camera waits before moving after the mouse is moved. Best
