Good game engine, often feeling like a multiplayerversion of Half-Life, but 3 times better. Broad support for modding and addons, and plenty of those to be found on the web. Good fluent shooter gameplay with some dynamic stuff i.e.: items and pick-ups placed randomly (still at pre-defined locations though). Few glitches and a good overall stability. The game content is mostly gore not horror, nothing really scary in there just disgusting.
Sadly the gaming community is the worst of all i encountered in any multiplayer game. There are players to have a good game with that is roughly 20-30% of the average gamer population for this. The rest of them are egomanic loners who love to waste your gaming experience by either rushing, sabotaging, or even outright teamkilling you. The former are the most frequent type, making you wonder why they even bother playing this with others when they would be much better off playing the game single player. Often you will find they set up fake accounts to "troll" those who simply wish to enjoy an imersive campaign and play with a good team. (Votekick usually wont work, so just let them rush and wait until they get incapped and killed by a special infected or tank, then they usually leave because you did not save their royal butt.)
So, if you get this either have 4 or more friends you will be playing it with or be prepared to play this singleplayer more frequently than multiplayer. On a bright side the entire game can be finished in singleplayer and AI is okay as well, just a bit stupid-ish.
