You're Doing It Wrong achievement

Can somebody explain this achievement to me?
It says "Complete the game without activating an item" - what exactly is meant by "an item"? Those things I need action points (fists) to use?
'N' somewhere I read that I have to use an item ONE time - right now I have to "Shatter the Ritual Stone" with the "Hammer of Righteousness"...

You can't use any of your own items, like swords or potions, but you can use the special items that you have to use for some boss battles. It's been awhile since I played so I cant' remember but I think there's some special item that does 999 damage and the boss is resisitant to all spells so the only way to win is to get enough action points and activate that item. But in just a regular battle you can't activate your items.

You will be able to use only 2 items - Hammer of Righteousness and Molten Metal. That`s all. No bullfighter cap, no water bucket and torches.
