2. If possible, stay behind walls while healing to avoiding heavy fire.
3. Don't go running around with no patient, the Medic has very low survivability.
4. Pop Ubers on people when they're ready to be popped, don't go holding on to them. They recharge, no reason you should hold onto them.
There are some of my tips.
Spies will often target you and will often stalk you in disguise for a time whilst trying to catch up with the faster Medic. Beware teammates with their primary weapons equipped and not firing or the wrong weapon equipped for their situation (ie Pyro's with their axes out). Spies can drop down (and often do this) large cliffs or off buildings to drop-stab a medic. The cliff on Doomsday and the structures on Harvest are notorious drop-zones I find. Try to avoid these if possible as it's hard to simultaneously keep that full LOS to potential situations and look up for spies also.
A full awareness of what's going on around you as a medic also allows you to learn from people playing their classes and their positions, see potential problems coming up and get heals to that burning teammates just in time.
Whenever your character jumps in the game, you're basically telegraphing your head's future position to the sniper. It's really easy to headshot people when they do this (unless they abuse crouch jumps), only jump when you're being attacked by soldiers or demomen! Be unpredictable and get out of that sniper's LOS!
Finally, when possible try and back your character against a wall. This limits the space you need to be aware of by 180 degrees and helps you focus on what's happening more.
