The actual part that's worth is the lost of fall damage.

the pistol basically negates one of the primary advantages of a few other unlocks. with the pistol, your maximum base health is 140. No weapon in the game does that mouch damage to you unles the hit is a critical. There are a few weapons that can do 125hp or more damage to you though, like the direct hit and loch 'n' load.

Situational, If the enemy team has 2 or more pyros, I don't recommend using it, but in all other situations, I recommend it.

I main it w/ scout. No fall dmg, ever. Extremly usefull unless you are on a CTF map.

Too bad it doesn't remove fall damage in the nucleus pit

the loss of falldamage pairs really well with the increase of pushforces taken from
the shortstop also the extra health you get from the shortstop combines with the pocketpistols higher health.
They shoot from the same ammo pool though :/

Why would you be using the PBPP in combat, anyways?

you shouldnt be letting a pyro close enough to airblast you in the first, the draw back is negligible

it allows you to carry the sandman without hp lose
also fire damage isn't important coz pyro kills you in many cases

It's worth it sometimes. Depends on the number of scouts and pyros on the other team.
