When you do quickscopes* some classes can survive a headshot.
* You zoom in and nearly instantly shot, you don't wait for the charge to be full.
On-topic: If you zoom in far too quickly, theres a fraction of a second from zooming in that prevents headshotting until that fraction second is over. DDS allows him to barely survive a single headshot from full health with the extra 25+ health given and 15% bullet resistence, but takes 20% more damage from explosives.

The headshot immunity has been removed.
Doing headshots with the Sydney Sleeper is not possible.
But i see there are server plugins which bring it back to life.
Vaccinator gives out minor (MAJOR when Uber) resistance to specific type of damage (in this case, bullet damage)
Darwin Danger Shield gives out a minor resistance to bullet damage, effectively stop quickscope from killing the wearer.
Ah yeah, they are not pretty up do date. Stick to the english one and when you play the game regulary you might looking on on the official tf2 site, they update 1+ times per week ;)
Actually, Vaccinator currently has a bug which blocks critical hits of selected damage type even during the regular healing, not just during übers as it was presumably meant to function. This means that headshots on Vaccinator'd targets only deal damage of an equally charged bodyshot minus the 10% damage resistance to selected damage type while healing.
