Almost any I Wanna Be The Guy fangame (or the original, obviously) would fit the criteria for sure. They're also all free so just search them out if you're interested. I'd personally recommend I Wanna Be the Fangame and I Wanna Be the Boshy (hardest one I've played) as the fangames go. Of course the original and IWBTG Gaiden are worthwhile as well. It takes a certain mindset to really be able to enjoy those games, though.

No problem, also if you haven't played The Binding of Isaac you might enjoy that one as well. Not as much of a reflex kind of difficulty as SMB but it's certainly not an easy game to master either.

There's also Dark Souls, which I wholeheartedly recommend as my favorite game ever, and I don't say that lightly. That said, while DS is a punishing game, the difficulty is largely from having to figure everything out on your own and not so much on technical controller skills. Still, the first times through that game can be really harrowing.

The Binding of Isaac is the n°1 choice.
If you want to try before you buy, I suggest you take a torrent in order to see if it's worth paying the price.

Super Hexagon, maybe? Very different from all the games mentioned here but I promise you it's very challenging and lots of fun.

I'm playing Grimrock right now, it's really tough on normal, I think you'd really be up for a challenge on hard difficulty + hardcore mode (no auto-map).

Another one, which will be quite frustrating, VVVVVV. You can try the Demo on Steam, it's somewhere here.

I highly recommend Spelunky & VVVVVV. Spelunky is done in a roguelike format though, meaning you have to survive the entire game rather than restarting at a checkpoint over and over until you pass. You may like it. You can download the freeware version or buy the XBLA version if you have a 360.
