So what do I do after I finish the game?
Is there any way to continue on and explore the places I haven't seen, or do I have to start from scratch?

You'll need to start over. Once you leave a mission area or a hub, you don't get to go back and clean up side quests. You do return to Detroit and Hengsha for a second visit each, but the available side quests (and in some cases, areas that you can visit) are different.

I kinda hate games that take away all your hard earned stuff away from you, but it's the ending so it makes sense anyways.

Not with the original, but the "Director's Cut" has a "New Game+" that allows you to begin a new game with the augmentations you gained in the previous game. It's not my favourite thing as it makes the early going too easy. But, the option is available for those interested.

Is New Game+ the same, except you have your augs from the previous playthrough? Or is the game actually harder? (like harder hacks, tougher/more enemy patrols etc)

No, in most New Game+'s everything is exactly the same.

After i finished the game for the first time I just replayed and found a lot of things I had miss.
