What was the thing you get after the Kapala, and what does it do?

Nothing.

There's no other item. However, if you sacrifice two damsels worth after getting the Kapala, you will feel invigorated and gain 8 HP. But after that, there's nothing,

They also splatter for a lot of blood which fills your kapala.

Also keep in mind that the favor you get from a sacrifice depends on what you sacrifice and if it's alive or dead. A sacrifice that is dead grants you half of the favor compared to when sacrificed alive. A damsel for example is 8 favor. I don't know all figures but I assume you can easily find it on the spelunky wiki.
