I think you have to complete the game on hard and then very hard.

Or just skip that like me and enter the konami code.

Doesn't have to be the main story either. Beating Sam's story on Very Hard unlocks Reveangance for everything as well.

that was the original code, it should be the same with an xbox controller. Not sure about others.

B and A for Xbox controller, O and X for PS3. Unfortunately, the code can't be input with KB+M.

Beat the game on Very Hard.
To unlock Very Hard, beat the game on Hard.

you just need to complete the game on the difficult-level you like.
I completed the game on Easy and i could choose the Revengeance-difficult if i want

Lol I did, I'm one of the few with the all S rank achievement, I regret every moment of trying to get it. But I got it.

Lol I did, I'm one of the few with the all S rank achievement, I regret every moment of trying to get it. But I got it.
I got it on BOTH PS3 and PC :P

Necroing, but getting 100% completion isnt really that hard, it just takes up a load of time mostly
