Buggy death how do I report and possibly restore game?

I was playing Hiver on Insane difficulty. I was moving into the security station room and there was one Skunk Ape. I proceeded to melee him down with my Kheraizen, my health pool was around 35-40% (not wounded by any means), and I was very safe from death. All of a sudden I die and on the summary screen it says I died from an MK3 bot which is IMPOSSIBLE because there were no MK3 bots in the vicinity at all.

This was a very very good Insane Hiver run, one I have been working on for months and this death made me rage hardcore, not because of the usual "the pit BS nonsense" shenaningans, but legitimately I got a bugged death. Is there any method I can talk to someone at Kerberos to restore my game, or at least investigate the actual reason why I died? Because I'm not playing this game anymore after that stupid death. I've had enough stupid deaths due to RNG or the usual "the pit being the pit" nonsense, but this death was not only buggy but seriously the last straw.

There had to have been one hiding in a corner or something.

I have 861 hours on record, and I've never ever seen a random death, from an enemy that wasn't there.

Sad to hear about the unfortunate end on your (until then) prosperpors run.
May I ask what floor you was on?
Props for getting far with the Hiver btw.
I did my first play with him some weeks ago. Also my first game with any class on Insane.
I thought it went pretty well but eventually died at floor 15.
He is a tough class to play for sure.

I know nothing more about what happened then you but about how much HP did you have? If you had several hundered HP then a single Mk3 could perhaps not have killed you with either shots or Greanades (on a single turn). But not sure the maximum output of that Monster per turn. Anyone knows that? More then 300HP (with 40 armor)?
Also depends on your armor ofc. If you had one equipped but the 40 natural armor on the Hiver makes a significant reduction in possible damage too.

I have not encountered a "bugged" death either. Or heard of it.

It was buggy, but I figured out my death. It said I died from an MK3 bot but really it was from stat drain. I didn't realize I had still my Slave Collar on and I assumed my power stat went below 1. So when the Skunk Ape did his poison mist and poisoned me, I must have died from that.

I definitely got tilted and really confused from the result screen when it said MK3 was the result because the room was tiny AND in a corner so there was no way it was possible. Either way, still gonna truck on with the warrior.

Farthest was floor 31 or 32 so far but the ammo crates were scarced and really bad and even Belt of Aias couldn't carry me. Also lost my Kheraizen and Ri due to overuse and not enough repair stations. And no armor lockers either.
