2. I also never really see staff members use the bathroom. Make sure the area they're supposed to use is marked as staff only. If it already is, you have a bug. Staff needs have been bugged for a long time.
3. Happens on big prisons. I have one with 900 prisoners and it happens. Nothing you can really do about it.
BONUS: An annoying feature, but it's not really a bug, so the devastating won't fix it.
You should email the developers this list so you get their attention. I'm not sure if they monitor this forum.

Improve your next prison design to be able to deal with it. This game is about building, and building again, improving your design by learning from what didn't work out in your last prison. A few prison designs further down the road will look much better if you do it right. And then you will look back at your current prison and think 'omg wtf was I thinking'. :)
Ai routing not being based on proximity is an old motherbug, this is why having several staff rooms sounds nice but won't work as expected, so stay away from that idea, same with kitchens. Learn from it, improve your next prison design to have one efficient central staffroom to deal with this problem. No job / needs fulfillment is based on proximity, they just walk off to random areas. Steer your staff/visitors/prisoners via more efficient prison design.
Check Deliveries and Storage, they might be completely filled, making it impossible for items to spawn when needed, leading to snowball effect. Also check for boxes/cablestacks in walls, if they can't reach it then they are unable to put it down elsewhere. If you want extended info on what's going on, then start a game with Devtools/Cheats enabled. Press F9, and have a look at the Navigation, Obstruction Grid, and Jobs. You might see the cause of your problem more easily with these tools. To enable this on your current map, edit the savegame with Notepad++, put the line 'CheatsEnabled true' on top where all similar options are specified.
Use F9 to see why. BTW, F1-F11 also do things, just don't press F11, it will expand your land in all directions. Or was it F10? can't remember...
Might have been a fight / somebody braking down his cell just before that moment without you noticing it, load/restart should solve it.
Deal with it. Either play hard, live with the riots and have enough guards/snipers/armedguards, or play reform and remove punishments for incidents, or dont bother, give him a sweet cell out in the cold forest brick his door and conveniently forget about him. Or use my teslamatic 2.0 mod to fry them if you had enough of them.
I've been dealing with this issue as well. I realized that at least in my specific circumstainces, there was a significant improvement when I installed more sinks and hired more cooks. Having that said, I have no knowledge of bugged tray stacks. I either don't have them, or they don't have a noticeable effect in my prison.
Regarding food and clean trays - like I said, I believe hiring more cooks and installing more sinks should improve things considerably. Regarding toilets, bladders and bowels - I can't exaplin it, as I don't have this issue. My staff usually do their stuff when they need to. Maybe you should try installing toilets on armories, kennels and security rooms as well.
The only issue related to staff needs I really can't find a solution to, is warmth. The absurdity is the most obvious with gardeners - they work outside by definition, yet there's no way that I know of, to keep them warm. If anyone has a clue - please let me know, as I'm completely clueless.
Yup, a nasty, annoying bug. Reload every time it happens is the only solution I'm aware of.
Get my Sauna mod. It's the only thing which perfectly helps, since staff doesn't warm up fast enough during their break in a normal staffroom. Especially snipers and dog handlers who stay out in the cold and have this need exploding will be very grateful to you. :)
Thanks, but as of now, I'm still optimistic about getting the last two achievements, and I suspect installing mods might prevent me from doing that. As soon as I either get those achievements or give up, I'll gladly install your mod. :)
