Also, avoid the secret levels, they are a death trap.
Honestly though...plutonia pissed me off with some of its tactics, namely sticking you into spots immediately surrounded. Doesn't matter if its a level start, teleporting in the middle of a ton of zombies, or walls falling down all around you revealing a hoard : Those situations are more hp / armor / energy cell dumps than a challenge. Do you have health, armor, and bfg cells? You survive. Do you not? ded. Kind of lame.
Its not so bad if you have even a second to react / run for cover, but when you literally cannot avoid 100+ damage being thrown your way, it feels stupid.
Most people won't know how they work unless they played it before or read up prior =p. Even as an old school Doom player (always played on my grandpas 386 lol) Plutonia is pretty damn hard. Which is awesome =D.
