Question about the Tie achievement

The tie achievement and the 100 deaths one are the last ones i need to 100% Octodad achievements (I got number one dad very recently) and i'm just wondering if you need all the ties in the shorts to get the achievement or do you only need the ones in the main story?

It's now required to get the ones in the shorts too for the achievement.

Some hints from one of the designers on the team:

Restaurant 1: A modern kitchen needs to be properly ventilated.
Restaurant 2: This tie is pretty saucy.
Restaurant 3: Mind your table manners.

Hospital 1: A good sailor should always be ready to fight off scurvy.
Hospital 2: Make sure your prep room is well stocked.
Hospital 3: If only Octodad could truly look inside himself.

Got all the ones in the Date and two of the ones in the Hospital, i believe the one i'm missing is "If only Octodad could truly look inside himself" at first i thought this had something to do with the X-ray so i scanned Octodad and it did make a photo, but i can't do anything with the photo, no tie.

No idea how to do this one, though i'm probably just missing something obvious.

Hmm, I believe it should have revealed a tie alongside the photo, unless you missed it.

For me, the tie pops out and then immediately disappears. I can't collect it.

When you play the level, did you ever have to reload from checkpoint (whether by losing or continuing the level after exiting the game)? Trying to figure out what may cause it to disappear immediately, but can't reproduce...
