Loot dissapering after patch?

Hello, some minutes ago I was playing and killed a hero monster and saw a legendary, "Grasp of Unchained Might, drop.

Then i saw the item dissapear, but thought that I had got it, cleared the rest of the enemys and checked my inventory and nothing.

Anyone else having this problem?

Edit: Found a solution. If I move out and come back the item was there, but as I got closer it disspeared again. Had to get far and the then click on the item to get it.

Sounds like it fell off the edge of the map to me. This means that it only shows up when your in the right spot.

Yeah, there are some spots on the map where it does that. Specifically, I remember an exact spot in the Warden's Cellar where that would happen. It was in a corner near a pillar, any items that dropped there would dissapear when you got close to them, and then reappear when you walked away. Happened every time in the exact same spot.
