Beardguy existed, but in Jacket's memory, he was faded, but still a major part. When Rat mask killed them, it was added up to the girl you saved dying, and your life falling apart. You knew it was him who killed her, Jacket put the pieces together before waking up in Trauma. And he knew Rat Mask had orders to kill your girlfriend because of all of the Endboss's men you killed.

also: it is unclear if all the stuff before Mission 12 really happened or if Jacket is just hallucinating or maybe remembering some stuff while being in coma.
i would say if beardman really existed he is probably still alive.. since Jacket is insane imo, which makes his perceptions of reality unreliable since he probably can't tell the difference between what's real and what isn't.
