WHAT?!
I had this problem and its really annoying, but i managed to fix it. All this is because the game have a fixed amount use of GPU memory, so if u have a better GPU the max memory the game will use is 600 when config the texture to use the "large" option, so do it:
-Set all ur preferences on graphcs like Antialinsg, ubersampling etc...
-Now go to the paste of game on docs, (C:Users//Documents/Witcher 2/Config)
-In the Config, open the archive User.ini and find the line "TextureMemoryBudget" (without the ").
-Now where u fix it, if ur GPU have like 2k memory, set it around 1800-2000,(ex: TextureMemoryBudget 1900) save and exit, now open ur game, on ultra, and it will be fine as hell!!
I hope it help, helped me at least, now i play with no lag.

Thanks for the tip. I have 7870 2GB, I set texture memory budget to 1500 and game works without lag and stutering.
