There are two gender specific characters, male characters get the Handmaiden, female characters get the Disciple. There are alot of dialogue changes based on gender (not just changing the pronouns). There are also alot of implied possible future romances but nothing ever comes of this.

The amount of sexual tension in KOTOR 2 is positively insane.

I've never been a fan of the Disciple so I typically play as a male despite the canon exile being female. I'm not sure how the whole Disney retcon is going to affect any of the Old Republic era stuff.
