I played on easy and then in true west. In true west the HUD is minimal. You can't see most informations on screen, like remaining bullets. There's no indication of where damage is coming from. Also you don't have the crosshair anymore. Enemies have a better aim, I guess. Maybe you die with less shoots too. Enemies do not take cover better, though.

Honestly I'd say that True West felt very little harder than Easy. Probably because on the second round you are better used to the games mechanics. You keep all the upgrades too, which helps a lot. But I would say I had no trouble finishing in True West.

I really enjoyed playing on hard for my first go, but I did become frustrated during the last stage. I haven't tried True West yet, but think hard is probably the ideal form of the game. That said, if you're willing to replay it to get that experience, easy mode is a smoother way to enjoy the narrative for your first time.
