Just want to know which DLC people think is worthwhile, which isn't. It seems I already have a few, including the one that lets you make your own leader and some music and other cosmetics. No sword of islam, no patrician dlc, etc.

The game is playable well without DLC!

There are some DLCs that enhance the gameplay which is not bad also. Take a look at what interests you. – Maybe you would like to test the game before you try more content? Maybe you will have more motivation with a fresh enhancement in a possible replay later? However, the additions are a nice-to-have. Maybe Legacy of Rome adds some fundamental stuff here. – But it depends strongly on what and how you would like to play.

Music, Portraits and Units seem cosmetic… But I personally got all portrait-DLC because I thought that might help to recognize different persons in the game. And I think it helps a bit. Some say arranging marriages and such take about 1/3 of their play.

Unsure about the music. – One game can go on for hours and hours… But some music DLC just seems to get mixed into the main theme… I did not figure out much about it. I like the main theme, but one spends a long time with it.

Units don't make a difference in the gameplay. It's just a little nicer eye candy.

I look forward to the next big DLC "The Old Gods". The game will start earlier then and there are some changes that I would like to play ;)

Next to the DLC there are also interesting mods. Not available through Steam but made by the community. So there is more than enough to broaden the game ;D
If you are "through" with your interests, you can mod and cheat by yourself then! ;)

+1 with wooosh.
I would recommand you to play a full game without DLC, which should already be like 30-80 hours of fun.
Then you will see if you like the game, and you will have a clear view on what to get after. :)

The three that are most important right now are Sword of Islam, Legacy of Rome, and The Republic (in order of release). Legacy of Rome lets you make use of the retinue system (create standing armies) and gives new special events and decisions (like being able to blind prisoners). It's the only one of the three I mention above that makes a difference if you are playing as a Christain

The others I mentioned - Sword of Islam and The Republic - let you play as muslims or republican patricians (I'll let you guess which is which). I've actually not tried Republic yet myself, as that play style doesn't really appeal to me, though I'm sure I'll try it eventually. Sword of Islam, though, I've made extensive use of. So as others have suggested - first, play a good amount as a Christian first, and if playing as something else seems interesting after a while, then consider the DLC :)

Did the updates change something in the new DLC?

They made it so republics are not ridiculously overpowered.
