2) Player numbers are in flux, since the great disappointment of the new client. I've seen anywhere from twenty to two hundred players on at any time, previously. Currently, the new client does not show how many people are online, so there is quite literally no way of knowing.
3) Tokens are consumables to use on Champions. Champions range from Lv.1 to Lv.3, at Lv.1 being restricted to the default "upgrades" (these used to be entirely addative, but were changed for balance). At Lv.2, the first "upgrade" can be changed, and the second at Lv.3. It's very easy to get Champions to Lv.3 without using Tokens, especially if you use them for the Heroic Achievements.
4) The best way is to make a Battlegroup to farm Skirmishes (or PvE "Solo Games" now, I guess), typically using the Stealth mechanic (Thunderhoof is superior; don't let anyone tell you otherwise), and sacrifice any unwanted or duplicate (3+) Rune rewards in the Rune Forge to buy any pre-Expansion Runes for Shards. I would hold off on spending any Gold until we're sure when the pack update is coming; the difference in projected value is significant.
5) All are. However, there is supposedly a bug, last I heard, which prevents progress in the same campaign on different difficulties. So if you start Trials of Wrath on Normal, you can't start Hard or Legendary until you complete Normal... There is no difference in rewards, unless the campaign says otherwise; it's just for a little more challenge.

3) Game frozen glitch? End turn button does not work yet the game still functions and online connection is still on. Very annoying during a 50+ turn match.
4) I have finished a campaign and can restart it however I can not change the difficulty?
2) No idea; never seen "Dual-Alignment" before.
3) I've had that happen to me when I'm putting in the commands to win the match. You still win, but it just sits there. Other people have been talking about similar bugs.
4) Difficulty might just be bugged, in general, currently. There's really no community on Steam for the game, and all of the bug reporting is done on the forums: http://forums.poxnora.com/index.php

Pox Nora Release to Grimlic's Descent is Core.
Path to Conquest to the penultimate set is Extended.
Ultimate set is Expansion.
New sets are added every 3-9 months, depending on various factors. I'd imagine the new client release will delay the next set. Some sets are smaller and quicker, some are larger and slower. Bug fixes and balancing tend to come with them, as well.
