It's a mod for the original game. It is NOT part of the game, however, so it's pretty much a "troll" in terms of Cave Story+.

Not to sound like a broken record, but it is a troll video (and a really good one, too). I was actually almost convinced that it was real up until I saw the release date for the video and the comments.

It's not fake. The 1st post is right, it's a mod for the original Cave Story. It's not a troll nor fake, it's just that the modders won't bother modding CS+... Though I think there's one or two miscellaneous mods for it.

It may have been a troll video-
But the person who made it (HaydenStudios) made another mod called "Cave Story 2" that was an even BIGGER troll.

So either the 'mod/hidden code' was decided to release into the game, or they wrote it down as a april fools joke.

The fourth ending NiCALiS talks about is completely different. The fourth ending in CS+ is when you don't get back the Booster v2.0 from Momorin and head up to the fight the Doctor with the Mimiga Mask still on. So there are technically four endings but the creator of the original only counts three and the Mimiga Mask ending being something of a 3.5th ending.

The "fourth unique ending" is where you don't ever return the Mimiga mask and defeat the doctor with the mask on.
