How to Deal with Team Stacking?

I've been very busy since the starting of the new year, came back and found an decling game with players constantly trying to team stack. It doesn't matter how many good plugins are coded, people are still finding an way to ruin things for others.

How should intentional team stacking, which is ruining games for others be delt with? Just switching the stacker doesn't work anymore, as they reconnect under an different name. If you stop them from reconnecting temporarily, they come back later and do it again.
