Kinda involved but here it is.
1. Get a keyboard with a numpad or a usb numpad.
2. Own a mouse that allows setting of keystroke assignments to mouse buttons, like setpoint for Logitech mice.
3. Hook them both into the laptop having the issue.
4. When setting the keystroke for the mouse USE the numpad control for '+'.
5. Unhook all the extra stuff.
6. Make sure to leave the mouse software that let you set the keystroke assignment running.
7. Start CotN.
8. Enjoy not having the issue anymore.
Still kinda annoyed that this is still a problem they never fixed but for now I will just let the lapping waves of the Nile whisper to me an wash the stress away.

The '=' key didn't work on my laptop as well, but I find that pressing the 'fn' key and the numpad key substitute for '+' works perfectly fine.
It's likely that the game speed key is specifically programmed to work on numpads instead of the keyboard equivalent, so nobody's complaining when it's borked... except for us.
Hope this works for anyone with a similar problem - I've been researching this for most of the day and I'm finally settling in to enjoy the game. 0.5 speed can go to hell.
