question
just bought the game and i have a few questions:
2) how do i know what each item does?
3) some times isaac runs really slowly is this normal?

BoI is a flash-based game, so it's not optimized in terms of "game speed". Try lowering the resolution and/or quality (Options menu)

About the first question, just try it out each item and the combinations. (If you are feeling lazy about item hunt, just take a look into the Binding of Isaac wiki)

I always play with the Binding of Isaac wiki open, sure it's lazy but it has saved me quite a few times

The game is meant to be played several times. (Go read Edmund's blog)

1) The game is a roguelike so you only retain unlocks and achievments.

2) Stick to the wiki, It has some missing information to do with hidden information but thats something you will have to learn with experimentation anyway.

3) Because BoI uses flash it has no graphics acceleration. This means that unless you've got some chunky cpu cores or you computer supports hyperthreading like the i series it will likely run quite slow when it has multiple elements on the screen at once. I used to have problems with my quad core 2 ghz per core phenom computer.

Yes. When you complete the game, you also start all over. It has to be completed many times to get every challenge done, every item unlocked... hundreds of times, in fact. Yes, there is more after the Caves. In fact, depending on which patch and whether you have the expansion, there may be a tonne more. There is the Basement, the Caves, the Depths, the Womb, Sheol, the Celler, the Catacombs, the Necropolis, Utero, the Cathedral, the Chest, and I AM ERROR.

If you're going slow you might have gotten a speed-down item (like a pill) without realizing it. You can check your stats in the pause menu.

Just play with the Wiki open, the Trinkets you find tell you nothing about what they do and it's pretty hard to tell for most of them. Most of the passive items tell you what they do at the bottom right of the screen.
