Best spacebar item would be D6, (Candle trailing right behind it) and best passive item I honestly think is Polyphemus. Things like Doctor/Epic Fetus are just boring and easy.

I like Polyphemus because it synergizes so well with so many different things on top of being a ridiculous boost of your damage alone. (Great with Brimstone, Mom's Knife, Sacred Heart, Max's Head/Magic Mushroom, etc etc etc.)
Not to mention that slow and powerful shots are better than fast and weaker shots against Isaac and Blue Baby.

Things like Brimstone are cool and all, but add something like Polyphemus, and that really makes it fun.
