I was playing as Cain and found an Ankh on my way down. I killed it lives and died, then, as ???, jumped on the hole to Sheol.. but didn't get the D6 :(

I thought that worked before? what did I do bad?

You have to defeat It Lives as ???, not ♥♥♥♥♥.

Do you mean that I have to KILL It Lives, not kill him with Cain and then as ??? jump down to Sheol? I got told that dieing with an ankh made the game think you were ??? all along so you could stil get ??? achievements..

You should have died in It lives, revived as blu baby and killed it with blu baby, that unlock's the d6

Gotta kill It Lives as Blue Baby. If you died then reincarnated as Blue Baby then you don't get the achievement.
