How do I get the D6 in this game?

beating the Cathedral 6 times unlocks both the Polaroid and the D6 i do beleive

Once you beat mom for the first time you unlock the womb. Beat the womb (mom's heart is the boss of the womb) 9 times and you unlock blue baby. Beat the womb with blue baby and you'll unlock the D6.

There is, however, an alternate method. If you're lucky enough to get the ankh (an item that will respawn you as blue baby if you die, regardless of whether or not you've unlocked him) and keep it until womb 2, you can just die respawn and kill mom's heart withou technically unlocking blue baby and you still get the D6. That's what happened to me.
