So yeah, we all know that the story is a little weird at best and absolutely insane at worst, but what could one read into it? To me it seems that everything the narrator says is what happens in the "real world", while Isaac's travel through the underground are hallucinations, potentially influenced by his dead silblings.

Yes, dead children seem to play a major role, as disturbing as it is. In fact, "Maggy" might just be the most disturbing. It's rather obvious that all the unlockable "characters" are just Isaac putting on pieces of cloth/wigs. Now, there is what appears to be a dead fetus called "Maggy", implying that she died during childbirth or other disturbing things I am not sure I should get into. And then there is his mother presenting Isaac the wig, as if to replace the lost Maggy.

One could interpret that she lost several children, given the enemies and "silblings" you can find as support. Could that be the reason she became so overly religious and insane?

Actually those pics aren't in the same order as when you see them after beating the chest, at least not now ... the character interp is pretty good but I wouldn't agree with their story art interpretation.
