Are you sure? The only ways to get him are from defeating Steven the boss (unlocked after beating basement 25 times) or in a golden chest (which is very rare)

Is that even possible?
You would have had to get and use a joker card before you visited either the item room or boss room since the devil room and the angel room don't spawn naturally on basement 1.

Steven can be dropped in the item room.

You're thinking of "little steven" who can be found in treasure rooms. He's also only unlocked by defeating steven. You can check the wiki
