I still haven't done no-damage depths, that's the hardest for me. RNG always screws me over even if i have a godly setup.

i'm still 10 away but I can forsee my hardest being either the chest with eve or samson or the purist

I also think that it's pretty hard to beat the 'don't get damage'-achivements, but maybe one day I'll be so bored to try it another thousand times and then I'll probably beat them :D

You go dude! My hardest waaaas.... Pyro I think, or possibly Scred heart, neither of them would frickin´ spawn! Can no longer recall. ^^

I'm at 94/97 secrets, I know I still need to beat the chest as Samson and Eve, beyond that I'm not sure what I'm missing

Dark Boy was my last and hardest acheivement. Also hard for me was challenge purist and find item "fate". im get platinum god half hour ago :)

My hardest achievement was "The Left Hand" because I couldn't find ultra pride for a long time.
